 Proof of Residency Requirements

All families registering a new student with the Hopedale Public Schools must bring the following required documents to the administrative office at the school their children will be attending as part of the registration process.
All of these:
            Child’s original birth certificate, I-94 form, or passport                                                                  

            Child’s up-to-date immunization record (see below)

            Parent/Guardian’s photo identification

            A deed or mortgage payment dated within the past 60 days

               or a property tax bill dated within the last year

               or a current lease of section 8 agreement. 

By 2 Years

By Kindergarten

By 7th Grade

   3 doses of Hep B

   4 doses of DTap / DTP

   3 doses of Polio

   3 or more doses of  Hib

   1 dose of MMR

   1 dose of Varicella

   3 doses of  Hep B

   5 doses of DTap / DTP

   4 doses of Polio

   2 doses of MMR

   2 doses of Varicella

    3 doses of Hep B

   Booster dose of Tdap

   4 doses of Polio

   2 doses of MMR

   2 doses of Varicella

For more information regarding immunizations, please contact your health care provider or the Massachusetts Department of Public Health Immunization Program at 617-983-6800 or toll free at 888-658-2850 

 Two of these:

            A utility bill (not water or cell phone) dated within the past 60 days

            A W2 form dated within the year, or a payroll stub dated within the past 60 days

            A bank or credit card statement dated within the past 60 days

            A letter from an approved government agency* dated within the past 60 days

               Approved Government Agencies: Dept. of Transitional Assistance (DTA), Dept. of Revenue (DOR), Children and Family               Services (DCF), Dept. of Youth Services (DYS), Social Security, any communications on Commonwealth of Massachusetts               letterhead

Residency fraud is a violation of Massachusetts state law and is subject to per diem fines for every day that a student attends a school outside the district in which he/she legally resides.
